All Scouting participants have to complete a Health and Medical Record form annually.  Parts A & B are required for all Scouting events.  Parts A, B, C & D are required for any event exceeding 72 consecutive hours.

Please note that medical forms will be checked for Twilight Camp (Parts A & B) and Webelos Resident Camp (Parts A, B, C & D) by camp staff.   Click here for the Boy Scouts of America's official policy and FAQ.  The official form can be downloaded on the left or at the before mentioned link.
